Role Overview
- Deliver high-quality, patient-centered care aligned with Elara Caring’s mission
- Assess patient needs, develop care plans, and monitor progress
- Collaborate with interdisciplinary teams to optimize outcomes
- Adjust treatment plans based on patient progress and goals
- Supervise and guide support staff
- Maintain accurate and timely clinical documentation
- Promote physical therapy services within the community
Requirements
- Graduate of an APTA-accredited Physical Therapy program
- Current, unrestricted PT license in the state of practice
- At least 1 year of experience in a healthcare setting
- Willingness and ability to travel within the service area (up to 50%)
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to lift, push, pull, or support up to 50 pounds
