Role Overview
The Opportunity Alliance is seeking compassionate, organized, and dedicated Care Coordinators (Case Managers) to join our team-based case management model called Behavioral Health Home and make a meaningful impact in the lives of our clients.
Work closely with individuals and families, connecting them to vital resources, advocating on their behalf, and supporting their journey toward stability and self-sufficiency.
Work alongside a peer, nurse care manager, and medical/psychiatric consultants to provide holistic care coordination for adults with mental health and possibly medical needs.
Meet one on one with clients to assess and support their needs, while also having the support of our integrated care team.
Enjoy a high level of supervision and support, which includes both individual and group supervision. There is no on-call, night, or weekend expectation.
Ensuring high quality of work, with clinical management supporting work/life balance and manageable workload.
Requirements
High school diploma or equivalent with one year of relevant adult behavioral health work experience as an MHRT-1 required
Associate’s Degree in the field of mental health, psychology, or a related field (behavioral health or human services) or a combination of education and experience that qualifies the candidate to receive an MHRT-C certification preferred
Certification: MHRT-C certification or (level A or B with a strategy to receive full certification)
Must have experience doing direct service with adults who struggle with mental health
Prefer case management experience with adults who have serious and persistent mental illness and have a history of trauma, substance use, homelessness, poverty, chronic pain and serious health issues.
One year of experience required, two preferred
Must be able to successfully pass a criminal background, child protective service check & sex offender check
Must not be on the state or federal suspension and disbarment list
Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, frequent typing and lift and carry material weighing up to 25 pounds
Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work. A good driving record is required.
