Home Health Occupational Therapist – PRN

Role Overview

• Perform comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning.
• Document patient assessments and pertinent information in a timely manner.
• Develop a treatment plan with measurable goals based on patient assessment.
• Complete appropriate goal re-assessment and discharge planning.
• Provide occupational therapy, including selection and use of adaptive equipment.
• Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.
• Train and educate patients and family members or others support and document understanding of treatment plan and goals.
• Provide occupational therapy according to standards of care defined by the state practice act and other professional and governing agencies.
• Supervise Occupational Therapist Assistants and Home Health Aides.

Requirements

• Bachelor's Degree
• Occupational Therapy
• Required
• 1 year Occupational Therapy Experience
• Required
• 1 year Home Health Experience
• Preferred
• Occupational Therapist (OT) State Licensing Board
• Required
• Cardiopulmonary Resuscitation (CPR)
• American Heart Association
• Required
• Driver's License
• Required
• Auto Insurance
• Required
• TRAVEL IS REQUIRED: Up to 75%
