Bilingual Analyst, Case Management – Field

Role Overview

• Assessing, planning, implementing, and coordinating all case management activities with members
• Manage a caseload of supportive and medically complex members
• Develop proactive courses of action to enhance member outcomes
• Determine appropriate services and supports for members' health needs

Requirements

• Bilingual in English & Spanish
• 1+ year of experience in behavioral health or a long-term care setting
• 1+ year of experience in case management and discharge planning experience
• Computer proficiency in Microsoft Word, Excel, and Outlook required
• Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in Melbourne and surrounding areas
