LTSS Service Care Manager

Role Overview

• Assists in developing, assessing, and coordinating holistic care management activities
• Develops personalized service care plans/service plans for long-term care members
• Evaluates the needs of the member and recommends/facilitates plans for the best outcome
• Coordinates between members and care provider teams
• Provides resource support for various needs
• Monitors care plans/service plans and provides recommendations
• Interacts with long-term care healthcare providers and partners
• Collects and maintains long-term care member information
• Performs home and/or other site visits
• Facilitates education to members and families/caregivers
• Provides feedback to leadership on opportunities to improve care and service delivery.

Requirements

• Requires a Bachelor's degree and 2 – 4 years of related experience
• Equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position
