Support and assist Case Management/Care Coordination/Transition of Care and resource utilization efforts throughout the LifePoint System to improve care processes and patient outcomes.
Collaborate and coordinate with the National Medical Director to develop and implement the Hospital Support Center (HSC) care management strategy for the Company and respective divisions of facilities.
Evaluate pertinent Case Management metrics within division facilities, identify opportunities for improvement, and facilitate actions to address the priorities at the facility or division level.
Conduct onsite Case Management programs assessments with facilities and potential new affiliates; Work with hospital Case Management and leadership teams to track, trend, educate/train, monitor, and develop effective corrective action plans
Maintain open communication with Health Support Center (HSC) division and department leadership team regarding performance of individual hospitals, gathering feedback for further improvement opportunities and strategies.
Collaborate with internal HSC departments to develop and present targeted education to optimize compliance with regulatory and payer requirements which promote high-quality, cost-effective care.
Requirements
Education: Bachelor’s Degree in Nursing; Master’s Degree preferred.
Experience: Minimum of 7 years of experience with 2-3 years as Director, Case Management.
Certifications: Certified Case Management (CCM) or Accredited Case Manager (ACM)
Licenses: Current licensure as a Registered Nurse in state of residence.
Ability to travel up to 50%
