RN Care Manager

Role Overview

• Works in partnership with patients, families, caregivers, and providers to deliver targeted care
• Collaborates with interdisciplinary staff and stakeholders within the care continuum
• Promotes patients’ optimal health through disease prevention and management strategies
• Applies evidence-based interventions to populations for cost efficient and outcome-based care
• Aims for positive outcomes including disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates

Requirements

• Bachelor's Degree (Required)
• APRN
• Advanced Practice Registered Nurse License
• State of Ohio Board of Nursing
• BLS
• Basic Life Support
• American Heart Association
• RN
• Registered Nurse
• Ohio Board of Nursing
• 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources
• Computer skill and data entry
• Knowledge and skill in clinical assessment and disease management
• Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.
