Provide care coordination and case management services for patients with acute and chronic conditions.
Serve as the primary point of contact for patients, families, and multidisciplinary clinical teams.
Support, educate, and advocate for patients, ensuring their individual care needs are met.
Coordinate care plans, monitor patient progress, and encourage adherence to treatment regimens.
Utilize EMR/case-management software to document and monitor patient care.
Collaborate with healthcare providers and insurance companies as needed.

Requirements
Education: A Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field is preferred but not required. Candidates with comparable experience or alternative degrees are encouraged to apply.
Experience: Experience in customer service, care coordination, or case management is preferred but not required.
Licensure: Nursing professionals with an active RN license are encouraged to apply, but licensure is not required for all candidates.
Certifications: Case Management Certification (CCM) is a plus.
Proficiency with EMR and/or case management software is beneficial.
Excellent communication, organization, and time-management skills.
