Requirements
- Unrestricted Registered Nurse license in Louisiana
- 2 years of experience in in-home case/care management
- Knowledge of community health and social service agencies
- Experience using electronic medical records
- Must document assessments electronically using mobile devices
- Engage members during in-home visits
- Autonomously work within the role and scope
- Must reside in Louisiana
