Role Overview
- Provide comprehensive care management for Disease Management programs
- Interact with beneficiaries to address clinical conditions
- Coordinate care with healthcare providers
- Collaborate within the Care Management programs
Requirements
- U.S. Citizenship
- Successfully receive interim approval for government security clearance
- Active RN license in state of employment
- 3 years of clinical or managed care RN experience
- Current CCM certification
- Bachelor's degree in nursing (preferred)
- Experience with Case/Disease management programs (preferred)
- Certified Diabetes or Asthma Educator (preferred)
- TRICARE experience (preferred)
