Managing a caseload of patients and providing comprehensive skilled nursing care in the home environment
Working closely with physicians and interdisciplinary team members to ensure safe, effective, and individualized care
Performing comprehensive patient assessments and developing individualized care plans
Conducting initial and ongoing skilled nursing visits based on patient needs and physician orders
Monitoring and evaluating patient status, including physical, psychosocial, environmental, and caregiver factors
Coordinating care with physicians, therapists, social workers, and other care team members
Communicating changes in patient condition and recommending appropriate interventions
Updating and revising care plans as patient needs evolve
Providing patient and caregiver education to support positive outcomes
Maintaining timely, accurate clinical documentation in compliance with regulations and company standards
Ensuring adherence to HIPAA and patient confidentiality requirements
