Community Health Worker – Independent Practices

Role Overview

• Work with multidisciplinary teams to identify barriers to patient health care
• Coordinate support services and community-based resources for patients
• Conduct intake interviews and identify non-clinical barriers
• Maintain accurate and timely patient records

Requirements

• High school diploma or equivalent
• Community Health Worker Certification by an approved training program or started within 1 year of hire
• Experience working with under privileged populations in the community
• 1-3 years of experience working directly with patients
