Role Overview

• Serves as the main liaison between the patient, family, and clinical team
• Coordinates, navigates, and schedules all aspects of patient care
• Participates in pre-visit planning for smooth operations
• Provides support regarding referral management and intake
• Coordinates complex appointments across patient continuum of care
• Conducts patient outreach and follow-ups on appointment needs
• Communicates using various platforms including telephone and virtual solutions
• Facilitates communication among clinical care teams and supportive services
• Creates a warm and welcoming environment for patients and families

Requirements

• High School Diploma/GED
• 2 years experience interacting with patients
• 3 years experience working in a complex health system environment or related field
• Ability to handle numerous tasks at the same time
• Superior customer service skills
• Working knowledge of basic computer functionality
• Effective and professional written and verbal communication
• Ability to maintain strict confidentiality
• Critical thinking skills and solutions-focused approach
