Role Overview
- assists in identifying members for high risk complications
- obtains clinical data as directed
- provides information collected to the responsible RN
- participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities
Requirements
- LPN/LVN, LSW, LCSW, or LMSW license
- Nursing Diploma or AS in Nursing or related field
- minimum of 2 years of experience in working with individuals with chronic illnesses
- strong preference for case management experience with older adults or individuals with disabilities
- must be fluent in English
