Perform reviews to ensure forms are completed accurately and timely
Assist in the development and maintenance of policies and procedures, training materials and job resources
Lead projects and participate in workgroups with cross-functional teams
Serve as a representative on business unit and enterprise initiatives
Perform process evaluations and tests and monitor systems/process enhancements
Responsible for overall management of member's individual service plan
Develop, monitor, and revise the member's service plan
Perform telephonic or face-to-face history and program needs assessments
Identify members with potential clinical health care needs
Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
Establish short and long-term service and support goals
Serve as a mentor or preceptor for new staff
Requirements
Requires a BA/BS degree
Minimum of 2 years of experience working with a social work agency
Minimum of 5 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role
Bilingual in Spanish/English preferred
BA/BS degree field of study in healthcare related field preferred
