Social Work Supervisor

Role Overview

• Responsible for the administrative oversight and supervision of direct reports at the assigned PACE site
• Promotes effective collaboration and individualized care planning amongst interdisciplinary team members
• Coordinates routine assessments and care plans
• Assists with family conferences and community referrals
• Monitors team member documentation, onboarding and orientation of new hires
• Provides guidance related to participants’ behavioral health concerns
• Collaborates with other site leaders to monitor site safety and overall staffing changes

Requirements

• A master's degree in social work (MSW) from an accredited university is required
• LCSW is preferred
• Minimum of 2 years' experience in a supervisory position required, in a health-related or human services field
• Minimum of 2 years of experience in case management/social advocacy/care planning with the geriatric population preferred
• Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines
