TAY Navigator, Multiple Positions

Role Overview

• Assess youth’s abilities, interests, and talents for academic and independent living goals
• Complete mandated documents and assessments for youth
• Ensure youth obtain and maintain required documentation
• Maintain ongoing contact with youth aligned with TAY policies
• Provide guidance and support for youth to navigate challenges
• Connect youth to community resources and programs
• Facilitate completion of NYTD survey for eligible youth
• Update Youth Summary and maintain electronic case records
• Collaborate with caseworkers, foster parents, and other partners
• Offer support for youth transportation for meetings and activities

Requirements

• A bachelor's degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences
• At least two years of college level coursework with 12 college credits in relevant fields
• Two years of equivalent work experience may substitute for education
• Knowledge of laws pertaining to foster youth
• Excellent organizational skills
• Strong interpersonal skills
• Flexible schedule availability
