Provides direct nursing services and nursing leadership direction for services delivered and outreach to support and optimize continuity of care in rural Nevada.
Works in partnership with the Practice Manager to assist with the team-based care model.
Responsible for overseeing the Home Visit Team, the schedule, travel, and activities to improve patient access.
Monitoring utilization and reporting of Remote Patient Monitoring Programs.
Assists with the assessment of medical support staff competency and ability to identify training opportunities.
Works closely with Population Health and Elko County Health Department to identify the targeted high-risk population within practice site(s).
Collaborating with all internal and external healthcare team members.
Coordinating and participating in any necessary in-office services or home visits to support patient access to care.
Triage walk-ins and phone calls at the health centers, where applicable.
Participates as a leader in the administration of NVHC programs, such as the immunization program.
Requirements
Graduate of an accredited nursing school
Licensure as a registered nurse in the state of Nevada
Current CPR or BLS certification
Minimum three years of experience in a clinical practice with good clinical skills/ability to function as a staff nurse
Active Nevada Driver's License
Bachelor’s degree in nursing (preferred)
Knowledge of the essential functions, practices and procedures of a medical clinic and office
Knowledge of in-office lab procedures; pediatric and adult phlebotomy skills
Knowledge and skill in pediatric and adult medication and immunization administration
Ability to interact effectively and positively with other staff members
Detail oriented and ability to handle multiple and shifting priorities
Excellent ability to problem solve, deescalate/resolve conflict and perform service recovery.
Demonstrated ability to produce high quality work in a consistent manner
Demonstrated ability to manage timelines and projects successfully.
