
Description
The Client Transportation Driver is responsible for coordinating with the shift lead to connect clients with the services they require. The Client Transportation Driver is often the first representative of Axiom Care that a client will meet and must be able to conduct themselves with professionalism and show compassion for our clients.
Shift: Full time - Days
Wednesday 1:00 PM - 6:00 PM
Thursday & Friday 6:00 AM - 6:00 PM
Saturday 6:00 AM - 5:30 PM
Duties/Responsibilities
· Pick up clients from hospitals, police stations, jails, other medical facilities, and Axiom referral partners, and transport them to the appropriate Axiom facility.
· Help orient new clients to Axiom facility – introduce the client to housing staff upon arrival to Axiom Care facility.
· De-escalate crisis situations as needed.
· If/when appropriate, draw from personal recovery experience to encourage clients to actively seek recovery.
· Safely transport clients to and from off-site appointments as needed.
· Transport clients post-treatment to their next treatment setting and ensure warm hand-off with receiving facility, whether another Axiom facility or not.
· Outreach and build relationships with representatives of Axiom’s referral partners, presenting self professionally.
· Assist with client intake process as needed, including property inventory, client orientation, and EMR onboarding.
· Act as a front-desk receptionist when needed by greeting visitors, answering incoming calls, and directing facility staff as needed.
· Performs other duties as assigned by management.

Education and Experience
Physical Requirements:
· Physically able to drive a vehicle for prolonged periods of time.
· Must be able to lift and move luggage weighing up to 50 pounds.
