What you’ll do
Lead support planning and coordinate Support Workers to deliver person‑centred care
Manage caseloads, balancing safeguarding, admin, meetings and team guidance
Support customers with daily living skills, including budgeting, cooking and appointments
Work with multi‑agency partners to safeguard, empower and support move‑on into the community

You have
Experience supporting others, ideally within supported housing
Ability to assess referrals and coordinate support
Self‑motivated, resilient and calm under pressure

The practical bits
Rota-based role, Monday–Sunday, with alternate weekends
Core hours with flexible shifts between 8am–10pm
Based mainly in supported accommodation, with community visits across Hartlepool
Enhanced DBS with barring list required (paid for)
Full driving licence and business‑insured vehicle required (mileage paid)
