Provide day-to-day management and oversight of Palliative Care Delivery teams
Lead interdisciplinary teams of Providers, RNs, Social Workers, and Medical Assistants
Develop and implement program strategies, goals, and initiatives
Drive execution and achievement of department KPIs
Assess palliative care referrals based on patient and family needs
Ensure delivery of coordinated, patient-centered care
Partner with department leaders across the organization
Requirements
Master of Science in Nursing (MSN)
Active, unrestricted Nurse Practitioner (NP) license required; ability to obtain California NP licensure within 90 days of hire (if unlicensed in CA), plus additional state licensure as needed.
