Case Manager

Role Overview

• Complete a minimum of 16 hours of training delivered by the Supervisor prior to beginning home visits.
• Visit with families in the family's home on a twice weekly basis, demonstrating reading, play and verbal interaction behaviors with two-year-old and three-year-old children and the parent or primary caregiver.
• Complete the required paperwork on a weekly basis.
• Attend and participate in weekly supervisory meetings, staff meetings, individual supervision and training, as is required.
• All other duties as assigned.

Requirements

• High school diploma or equivalent
• 5 years of experience working in human services required
• Bachelor’s degree in human services or related field preferred
• At least 1 year of prior experience working with young children and parents in a community-based organization is required
• Must have demonstrated knowledge of the community being served
• Proficient computer skills, including Microsoft Office, required
• Excellent interpersonal and communication skills, both verbal and written, required
• Ability to work independently and maintain confidentiality required
• Strong judgement and problem-solving skills, including knowing when to seek help, required
• Bilingual in Spanish preferred
• Valid and clean NYS Driver’s License required
• Travel is required
