Provides crisis intervention, information and referral and linkage to community resources
Coordinates clinical and financial outcomes of an assigned population
Thoroughly documents assessments and interventions in patients medical record

Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multi-disciplinary team and community agencies to achieve desired patient outcomes.
