100%, day/eve, Monday-Friday / 8:00am-5:00pm / Rotating weekends / Rotating holidays.
Your primary work location will be 2550 Charles St Rockford IL.
You may be eligible for a $5,000 sign on bonus

Completion of an accredited program approved by the American Physical Therapy Association. Required
Current license from the State of Illinois or in the process of acquiring licensure. Required
Valid State driver’s license and proof of liability insurance. Required
Bi-Annual CPR certification. Required
An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
