Provide centralized supervision and technical assistance to a team of Community Health Workers (CHWs)
Coordinate and collaborate with each CHWs on-site supervisor
Encourage a consistent, person-centered, and culturally responsive approach
Track CHW performance and help them meet goals for screening, navigation and care management
Build and maintain relationships with assigned community-based organizations
Provide project management support, helping with workflows, resources, and implementation plans
Support organizational change efforts so CBOs can adopt SCN processes
Participate in shared learning with Partner Liaison Specialists and the broader SCN team
Requirements
Bachelor’s degree in human services, Social Work or other related degree preferred
Equivalent experience in lieu of education may be considered
Minimum of 1-3 years' human services experience
Excellent verbal and written communication skills
Demonstrated ability to build trusting relationships
Exceptional customer service skills with commitment to helping others
Ability to quickly adapt and be flexible in approach to job tasks and challenges
Excellent time management skills with exceptional attention to detail
Basic computer literacy, including the ability to use email, conduct online research, and create basic documents (MS Office Suite including Excel, Outlook and Word)
