Coordinate care for members with chronic medical and behavioral health conditions
Conduct comprehensive assessments to evaluate members’ needs
Connect members with appropriate resources and support services
Develop and implement individualized care plans
Monitor member progress and advocate for necessary services
Collaborate with the interdisciplinary care team for optimal health outcomes
Document assessments and interventions in electronic health records
Participate in team meetings to discuss member status and care strategies

Active and unrestricted Registered Nurse (RN) licensure in the state of OH OR compact licensure in state of residence
Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
Minimum 3+ years of nursing experience
Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience
Experience or willingness to learn about care management within Medicare and Medicaid managed care
Familiarity with community resources and services
Confidence working at home / independent thinker
Bilingual skills, especially English-Spanish
Associate’s of Science in Nursing (ASN) degree and relevant experience in a health care-related field
