Role Overview
- Deliver comprehensive ambulatory care management services to identified patients
- Conduct care management screenings, assessments, and evaluations
- Develop and implement patient-centered care plans with shared goals
- Collaborate with healthcare providers and community-based providers
- Support proactive care for at-risk patients through all care settings
Requirements
- Current Registered Nurse (RN) license in state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution
- Demonstrated clinical nursing experience in chronic disease management
- Proficiency in basic computer skills and Microsoft Office software
- Caregivers must maintain current BLS certification and a valid driver's license
- Acceptable driving record and reliable transportation
