Acts as a liaison between Mercy Home Health and referral sources such as discharge planners in acute care, skilled nursing, short-term rehabilitation settings, as well as independent living, personal care and assisted living facilities, CCRCs and physician practices
Works across all departments as necessary to coordinate care
Works closely with the referral source, patients, and their families to outline services and gather necessary information for a safe transition to home
Responsible for processing referrals and for increasing market share from assigned accounts to achieve target forecast

Bachelor's Degree
3-5 years healthcare sales experience
Home care experience and/or clinical background preferred (i.e. Nursing, Medical Social Work)
Computer efficient with working knowledge of Microsoft Office Suite
Current PA driver's license required
BS/BA Degree in related field or business
Exceptional communication skills, both verbal and written
Ability to develop good rapport with referral sources and other community groups
Previous sales/marketing experience required
Experience in healthcare-related field with a proven track record
