Role Overview
- Deliver high-quality, patient-centered nursing care in home settings
- Collaborate with Case Managers to assess patient needs and coordinate care plans
- Educate patients and families about treatment options, medications, and self-care strategies
- Serve as a liaison between patients, providers, and community resources
- Monitor patient progress and report changes to the care team
Requirements
- Graduate of an accredited nursing program
- Previous home health/OASIS experience is a plus
- Active state LPN license, CPR certification, valid driver’s license
- Current physical exam and TB test
- Strong clinical, communication, and problem-solving skills
- Compassionate attitude toward caring for the sick and elderly
- Ability to follow directions, read, and write
- Home health and OASIS experience preferred
