Digital Case Manager

Role Overview

• Manage members with newly diagnosed oncology conditions and Type 1 Diabetes
• Ensure timely and effective interventions and comprehensive health education
• Coordinate multidisciplinary resources and referrals
• Provide consistent follow-up to support optimal outcomes
• Oversee complex cases to ensure effective implementation of interventions and utilization of benefits

Requirements

• Registered Nurse (RN) with an active license in Tennessee or participating state
• 3 years of clinical experience required
• 5 years of experience in the healthcare industry
• 2 years experience in IDD required for Select Community
• Excellent oral and written communication skills
• Basic Microsoft Office and E-Mail proficiency
