The RN Care Manager serves as a liaison among Brookdale teams, residents, families, and healthcare providers within our communities, promoting coordination, communication, and collaboration at Brookdale’s HealthPlus communities.
Works closely with primary and specialty care providers to formulate resident-specific care plans.
Serves as a resident advocate and contributes to improving service delivery.
Educates residents and families about care pathways.
Coordinates services to enhance wellness and manage chronic conditions.
Provides advanced care planning education and ensures completed Advanced Care Plans for residents.
Applies quality improvement methodology to analyze and manage outcomes.
Requirements
Graduate from Accredited Nursing School and a minimum of 5-7 years of relevant experience required.
Knowledge of value-based care models is preferred.
Experience working in SNF and HH environments preferred.
Previous Care Management, Case Management, or Care Coordination preferred.
RN license required.
