We are currently seeking a compassionate, organized, and team-oriented Licensed Vocational Nurse (LVN) to join our hospice team in a Part-Time role serving the San Francisco Peninsula.
In this important role, you will collaborate closely with the Director of Nursing, interdisciplinary clinical team, and administrative staff to ensure coordinated, patient-centered care. Your work will directly support our mission of providing comfort, dignity, and quality of life for patients and families during hospice care.

The Hospice LVN provides skilled nursing care to hospice patients in accordance with the patient’s individualized plan of care. This role focuses on monitoring patient conditions, supporting comfort and quality of life, and communicating effectively with the interdisciplinary hospice team.
Provide nursing services within the LVN scope of practice in accordance with the patient’s plan of care and in coordination with the interdisciplinary hospice team
Implement treatments and monitor patient responses to care
Observe, document, and report any changes in patient condition to the Hospice Clinical Director
Complete timely electronic documentation in Kinnser/WellSky using company-provided devices
Follow all agency policies, procedures, and regulatory guidelines
Support patient and family education as appropriate
Perform additional duties as assigned to support quality patient care
Active California LVN License
Minimum one year of nursing experience
Hospice or Home Health experience preferred (not required)
Current California Driver’s License and reliable transportation
Strong communication, organizational, and teamwork skills
Ability to work independently in a field-based setting
