Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact attempts
Facilitate and support start-of-care (SOC) processes by confirming patient presence, availability, and readiness for initial evaluation
Serve as a liaison between the patient and the home health care team
Assist in verifying patient demographic and insurance information
Communicate findings in real time to the SOC Operations Team and others
Perform basic assessments within EMT-B scope of practice
Coordinate with Clinical Manager when a patient is found in acute distress
